Механическая поддержка кровообращения в лечении детей с терминальной сердечной недостаточностью

Аннотация

Механическая поддержка кровообращения за последние десятилетия стала одним из наиболее эффективных средств лечения больных с терминальной сердечной недостаточностью. К сожалению, в педиатрии данный метод в основном используется для кратковременного подключения, и к нему относятся прежде всего экстракорпоральная мембранная оксигенация и левожелудочковый обход с помощью экстракорпорального подключения центробежных насосов. При этом применение данных методов у детей позволило значительно уменьшить смертность в листе ожидания на трансплантацию сердца. На сегодняшний день практически единственным методом длительной механической поддержки кровообращения у новорожденных и детей младшего возраста является система паракорпорального подключения искусственных желудочков EXCOR, применение которых связано с большими проблемами тромбоосложнений. Имеется относительно небольшой клинический опыт использования полностью имплантируемых систем, разработанных для взрослых, у детей старшего возраста. Поэтому, как показано в настоящем обзоре, задача создания систем длительной поддержки кровообращения для новорожденных и детей младшего возраста на основе имплантируемых насосов является актуальной.
