Современные представления о критериях селекции доноров сердца
Аннотация
При ограниченных возможностях имеющегося донорского пула и одновременно растущих потребностях в трансплантации донорского сердца встает вопрос о расширении критериев отбора донорского сердца, как одного из способов повышения доступности трансплантации органов, и донорского сердца в частности. Использование доноров с расширенными критериями, с одной стороны, позволяет увеличить число трансплантаций и сократить время пребывания в листе ожидания, с другой – повышает риск неблагоприятного исхода трансплантации. Соответственно, доноры высокого риска нуждаются в более тщательной объективной оценке с использованием прогностических моделей, а органы, полученные от доноров с расширенными критериями – в оптимальном подборе пары «донор–реципиент». Анализ мировых и отечественных исследований, приведенный в данном обзоре, раскрывает глубину современной проблемы селекции доноров сердца.
