За период с мая 2012 г. по март 2013-го было выполнено 3 такие процедуры реципиентам, нуждающимся в выполнении ургентной ТС и находящимся в клинике в статусе 1В по UNOS на инотропной поддержке, имеющим крайне неблагоприятный прогноз выживаемости без выполнения ТС. В одном случае было пересажано сердце после реконструкции ревматического митрального стеноза, а в двух других – после коррекции дегенеративной митральной недостаточности. Во всех трех случаях коррекция митрального порока была дополнена аннулопластикой трикуспидального клапана. Все оперированные пациенты были выписаны из клиники в удовлетворительном состоянии. Заключение. Учитывая удовлетворительный непосредственный результат продемонстрированных наблюдений и наличие значительного опыта выполнения реконструктивных клапанных операций у кардиохирургических больных, ожидаемо увеличение пула приемлемых донорских органов за счет либерализации критериев отбора и возможности выполнения инновационных процедур реконструкции клапанного аппарата ex vivo. </p></abstract><trans-abstract xml:lang="en"><p>Aim of study. The demonstration of our own experience in heart transplantation after valve reconstruction. Methods and results. From May 2012 to March 2013 3 mitral valve annuloplasties were performed to recipi- ents, requiring urgent HT and having extremely unfavorable prognosis of survival without HT. The recipients were classified as United Network for Organ Sharing (UNOS) IB, and all of them were on inotropic support. In one case, HT was performed after reconstruction of rheumatic mitral stenosis, in two other cases – after dege- nerative mitral regurgitation. The technical aspects are reviewed of ex-vivo mitral and tricuspid valves repair with concomitant heart transplantation. All patients were discharged from the hospital having excellent postope- rative recovery. Conclusion.
