Дисфункция правого желудочка во многом определяет высокую летальность при левожелудочковом обходе. Одним из эффективных методов решения данной проблемы является применение устройств для правожелудочкового обхода. Рассмотрены основные физиологические механизмы взаимодейс- твия правого и левого желудочков сердца, влияющие на функцию правых отделов сердца. Показана необходимость оценки функции правого желудочка до принятия решения о механической поддержке кровообращения. Приведены примеры определения предикторов развития правожелудочковой недо- статочности. Рассмотрены основные методы и устройства бивентрикулярной поддержки кровообра- щения. </p></abstract><trans-abstract xml:lang="en"><p>The paper presents a review of the problems of mechanical circulatory support with left ventricular bypass in total cardiac failure. Dysfunction of right ventricular defines the high mortality on left ventricular device. One of the effective methods for solving this problem is the use of right ventricular assisted devices. There are considered of the basic physiological mechanisms of interaction between the right and left ventricles of the heart, affecting on the function of the right heart. Shows the need to assess right ventricular function before deciding on mechanical circulatory support. Provides examples of the estimation the predictors of the right ventricular failure.
