Доказана возможность эффективной коррекции полиорганной недостаточности и увеличение продолжительности жизни пациентов с критической сердечной недостаточностью при использовании устройств вспомогательного кровообращения. В данном сообщении мы приводим случай имплантации бивентрикулярного вспомогательного устройства кровообращения «Berlin Heart EXCOR» в качестве моста к ортотопической трансплантации сердца пациенту с дилатационной кардиомиопатией, критической сердечной недостаточностью и высокой степенью риска фатальных осложнений. Несмотря на тяжелый, длительный послеоперационный период после имплантации устройства «Berlin Heart EXCOR», протекавший с явлениями обратимой полиорганной, сердечной недостаточности, дисфункции насосов бивентрикулярной экстракорпоральной системы, удалось достигнуть стабилизации состояния больного, регресса явлений сердечной недостаточности и через 9 месяцев выполнить ортотопическую трансплантацию сердца с хорошим отдаленным результатом.</p></abstract><trans-abstract xml:lang="en"><p>Patients with terminal heart failure, refractory to drug therapy, are severe category of cardiology. The possibility of effective correction of multi-organ failure and an increase in the life-time of patients with critical cardiac insufﬁ ciency has been proved with the use of biventricular assist device (BiVAD). In this report, we present the case of implantation of the BiVAD «Berlin Heart EXCOR» as a bridge to orthotopic cardiac transplantation to a patient with dilated cardiomyopathy, critical heart failure and a high risk of fatal complications.
