Оценка возможности протезирования клапанов сердца у реципиентов почечного транспланта- та с позиции безопасности для функции трансплантата. Материалы и методы. Протезирование кла- панов сердца при функционирующем трансплантате почки с удовлетворительной его функцией было выполнено 5 пациентам. Средний возраст пациентов, среди которых было 2 (40%) мужчин и 3 (60%) женщин, на момент проведения операции на сердце составил 38,8 ± 12,6 года. Интервал между почеч- ной трансплантацией и операцией на сердце составил 40,3 ± 44,1 (от 2 до 120) месяца. До проведения трансплантации почки все пациенты находились на заместительной почечной терапии программным гемодиализом в течение 50,2 ± 48,6 месяца. У 4 из 5 пациентов причиной порока сердца явился ин- фекционный эндокардит. Результаты. Среднее время ИК составило 81,2 ± 21,7 мин, среднее время пережатия аорты – 63,6 ± 20,9 мин и гипотермия во время ИК – 29,2 ± 3,2 °С. Всем пациентам были имплантированы двустворчатые механические протезы «МедИнж-2» и «СarboMedics». Все 5 пациентов в удовлетворительном состоянии выписаны из стационара. Средняя длительность послеоперационного периода составила 14,2 ± 3,4 суток. У всех пациентов отмечено относительно гладкое течение после операции, без инфекционных осложнений, с удовлетворительной функцией почечного трансплантата и протезов клапанов сердца. В отдаленном периоде у четырех пациентов функция протеза и трансплан- тата удовлетворительная в сроки наблюдения 5 лет, 3 года и 6 месяцев после операции. Заключение. Наш опыт показывает возможность успешной коррекции пороков сердца в условиях ИК у реципиентов почечного трансплантата. </p></abstract><trans-abstract xml:lang="en"><p>Aim. Evaluation of the possibility heart valve replacement in renal transplant recipients from a position of safe- ty for graft function. Materials and methods. 5 patients, heart valve replacement was performed with a func- tioning kidney transplant at a satisfactory its function. The average age of patients at the time of cardiac surgery was 38,8 ± 12,6 years, among whom were two (40%) men and 3 (60%) women. The interval between renal transplantation and heart surgery was 40,3 ± 44,1 (2 to 120) months. Prior to kidney transplantation, all patients were on renal replacement therapy with hemodialysis program for 50,2 ± 48,6 months. In 4 of the 5 patients of heart disease was the cause of infective endocarditis. Results. Average time IR was 81,2 ± 21,7 minutes , the average time of aortic clamping 63,6 ± 20,9 minutes and hypothermia during CPB 29,2 ± 3,2 °C. All patients were implanted with double-leaf mechanical prostheses "MedEng-2" and "SarboMedics". All 5 patients in sa- tisfactory condition were discharged from the hospital. The average duration of the postoperative period was 14,2 ± 3,4 days. All patients had relatively smooth flow after surgery, no infectious complications, a satisfactory renal transplant function and prosthetic heart valves. In the late period in four patients and transplant graft func- tion is satisfactory in terms of the observation of 5 years, 3 years and 6 months after surgery. Conclusion.
