У больной в первые сутки жиз- ни выявлена высокая кишечная непроходимость. Выполнено иссечение мембраны двенадцатиперстной кишки. На 12-е сутки жизни выполнена операция Касаи. Со второго месяца жизни у пациентки выяв- лено практически полное отсутствие поступления желчи в двенадцатиперстную кишку. Выполнен хо- ледохоэнтероанастомоз по Ру. Гистологически подтвержден цирроз печени. По жизненным показаниям была рекомендована ортотопическая трансплантация печени. У пациентки диагностирован врожденный порок сердца: ДМЖП, ДМПП, осложненный легочной гипертензией, который требовал хирургической коррекции. Первым этапом выполнена операция пластики дефектов перегородок сердца в условиях ис- кусственного кровообращения. Пациентка была экстубирована в операционной через 25 минут после окончания операции. Ранний послеоперационный период протекал без осложнений. Через 11 месяцев после операции пациентке выполнена родственная ортотопическая трансплантация левого латерального сегмента печени. Пациентка выписана из стационара на 35-е сутки после трансплантации. Полученные результаты свидетельствуют о возможности выполнения коррекции сочетанного порока сердца и печени двухэтапным методом. Первый этап хирургической коррекции врожденного порока протекает без выра- женных последствий для функции печени и гемодинамических нарушений. </p></abstract><trans-abstract xml:lang="en"><p>Aim: Clinical case of successful two-stage surgical treatment of a 1-year-old child with congenital heart disease and biliary cirrhosis is represented in this article. At the first day of life laparotomy was performed because of high intestinal obstruction. Kasai procedure and Roux-en-Y choledochojejunostomy were per- formed on 12th day and at the end of second month of life, respectively. Liver biopsy showed the signs of biliary cirrhosis. At the same time ventricular septal defect and atrial septal defect with pulmonary hyper- tension were diagnosed. The first step of treatment was the surgical septal defects closure. No complications during procedure, cardiopulmonary bypass and post-operative period were registered. There were no nega- tive effects on liver function after cardiac surgery. 11 months later living-donor liver transplantation was performed without any complications.
