За последние несколько десятилетий в России и за рубежом наблюдается существенный прогресс в развитии трансплантации печени у детей. Стало возможным оперировать реципиентов с оченьнизкой массой тела, выполнять им трансплантации от не совместимого по группе крови живого родственного донора, а также выполнять одномоментные трансплантации с почкой и поджелудочной железой при сочетанных патологиях.</p></sec><sec><title>Цель</title><p>Цель: исследовать накопленный за 6 лет опыт трансплантации печени детям в ФГБУ «Федеральный научный центр трансплантологии и искусственных органов им. акад. В.И. Шумакова». </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. За период с мая 2008-го по июнь 2014 г. проведено 304 трансплантации печени297 детям в возрасте от 3 месяцев до 17 лет с массой тела от 4 до 32 кг. Выполнены 241 родственная трансплантация левого латерального сектора (в том числе 2 в сочетании с почкой), 8 трансплантаций левой доли печени (в том числе 1 в сочетании с почкой), 40 правой доли печени (в том числе 1 в сочетании с почкой), а также 9 трансплантаций целой печени от посмертного донора (включая 1 случай одномоментной трансплантации печени и поджелудочной железы) и 6 сплит-трансплантаций (4 сплит-трансплантации левого латерального сектора печени и 2 трансплантации расширенной правой доли). Среди пациентов в 41 случае был использован трансплантат от АВ0-несовместимого донора.</p></sec><sec><title>Результаты</title><p>Результаты. Проведены ретроспективная оценка хирургической методики и анализ результатов. Ранняя летальность составила 8,1%, годичное выживание – 86,4%, 5-летнее выживание – 78,8%.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты исследования свидетельствуют о перспективности данного направления органной трансплантации для детского контингента пациентов и требуют дальнейшей разработки и популяризации подходов в медицинской и социальной общественности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Over the past few decades in Russia and other countries there has been signifi cant progress in the development of liver transplantation in children. It became possible to operate recipients with very low body mass, to perform transplant from cadaveric and living donors with incompatible blood type, as well as to perform single-step transplantation of liver with kidney or pancreas in patients with combined diseases.</p></sec><sec><title>Aim</title><p>Aim: to investigate the 6-year experience of liver transplantation in children in V.I. Shumakov Federal Research Center of Transplantology and Artifi cial Organs.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From May 2008 to June 2014 304 liver transplantswere performed in 297 children aged from 3 months to 17 years with body mass 4 to 32 kg. 241 living donor liver left lateral section transplants (including 2 in conjunction with kidney), 8 left lobe (including 1 in conjunction with kidney), 40 right lobe liver transplants (including 1 in conjunction with kidney), and 9 cadaveric whole liver transplants (including 1 case of simultaneous liver and pancreas transplantation) and 6 split liver transplants (4 – split left lateral section transplantations and 2 extended right lobe liver grafts) were achieved.41 of these patients received a graft from AB0 – incompatible donors.</p></sec><sec><title>Results</title><p>Results. A retrospective evaluation of surgical technique and analysis of results were carried out. Early mortality was 8.1%, 1-year survival rate – 86.4%, 5-year survival rate – 78.8%.</p></sec><sec><title>Conclusion</title><p>Conclusion.
