Сочетанная трансплантация печени и поджелудочной железы является рациональной лечебной тактикой и в перспективе может стать стандартом лечения у пациентов с необратимыми заболеваниями печени и сопутствующим сахарным диабетом 1-го типа, так как известно, что сахарный диабет 1-го типа ухудша- ет отдаленные результаты изолированной трансплантации печени и является дополнительным фактором риска послеоперационных осложнений. Тем не менее, несмотря на постепенное расширение показаний к сочетанной трансплантации печени и поджелудочной железы и успехи, достигнутые в области транс- плантологии, данная операция остается редким явлением в мировой практике. В Федеральном научном центре трансплантологии и искусственных органов им. акад. В.И. Шумакова в мае 2010 г. впервые в Рос- сии была выполнена операция сочетанной трансплантации печени и поджелудочной железы от трупного донора 17-летнему реципиенту с основным заболеванием – цирроз печени в исходе аутоиммунного гепа- тита – и сопутствующим сахарным диабетом 1-го типа. В данной статье рассмотрены показания к сочетанной трансплантации печени и поджелудочной железы, различные методики операции и особенности посттрансплантационной терапии, а также описан клиниче- ский случай первой в отечественной практике сочетанной трансплантации печени и поджелудочной железы. 

Combined liver-pancreas transplantation is considered to be a reasonable treatment of patients with end-stage liver disease and concomitant insulin-dependent diabetes mellitus, so it may become a standard treatment of this category of patients. As known, diabetes mellitus negatively affects the long-term results of isolated liver transplantation and increases a risk of posttransplant complications. Notwithstanding the widening range of indi- cations for simultaneous liver-pancreas transplantation and advances of transplantology, this operation continues to be a rare event in world medical practice. In May 2010 in Academician V.I. Shumakov Federal Research Cen- ter of Transplantology and Artificial Organs there was performed a first in Russia simultaneous liver-pancreas transplantation to 17-years old recipient, suffering with liver cirrhosis (as a result of autoimmune hepatitis) and concomitant insulin-dependent diabetes mellitus. In this article we give an overview of indications for simultaneous liver-pancreas transplantation, different ope- rative techniques, immunological aspects and immunosuppression regimes, as well as we describe the first in Russian experience of such transplantation.
