ОПП отмечалось у 37 из 86 больных. ЗПТ применяли у 21 пациента. Основными факторами риска явились снижение функции почек до ТС, нарушение функции трансплантата, потребность во вспомогательном кровообращении. Основным методом ЗПТ была постоянная вено-венозная гемофильтрация, в 3 случаях начатая интраоперационно. Постоянные методы ЗПТ позволяют наиболее точно контролировать гемоги- дробаланс пациента. Расширение показаний к ЗПТ и увеличение частоты ее применения сопровождалось улучшением выживаемости пациентов. Наличие ОПП в раннем послеоперационном периоде существен- но ухудшало долговременный (более 10 лет) прогноз вне зависимости от глубины повреждения функции почек. </p></abstract><trans-abstract xml:lang="en"><p>Acute kidney injury (AKI) is a frequent complication after heart transplantation (HT). For the identification of risk factors of AKI and optimal modes of renal replacement therapy (RRT) 86 HT data was analyzed. AKI was observed in 37 cases. The main risk factors were renal failure before HT, heart transplant dysfunction and requirement in mechanical support. Continuous RRT was preferable due to the best control of patient’s volumes. The widening of indications for RRT was associated with better survival.
