В статье представлен анализ данных научной литературы последних 15 лет об осложнениях после ор- тотопической трансплантации печени в зависимости от тяжести исходного статуса реципиентов и вы- раженности метаболических и функциональных нарушений. Проведена оценка факторов, влияющих на развитие осложнений со стороны сосудистой системы (артериальный и венозный тромбоз), желче- выводящих путей (холестаз, холангит), иммунной системы (криз острого отторжения), а также хирур- гических и инфекционных осложнений и функциональной недостаточности трансплантата. Обоснова- на необходимость усовершенствования мониторинга в ближайшем посттрансплантационном периоде. 

The article presents analysis of scientific literature of the last 15 years about complications after orthotopic liver transplantation, depending on the severity of the initial status of recipient and expression of metabolic and functional disorders. Factors affecting the development of vascular complications (arterial and venous thrombosis), biliary complications (cholestasis, cholangitis), immune response (acute rejection), and also surgical and infectious complications and functional insufficiency of the graft are analyzed in this review. The necessity to improve monitoring in the immediate post transplant period is considered.
