Однако использование классического ИК повышает объем интраоперационной кровопотери и риск развития первичной дисфункции легочного трансплантата. </p><sec><title>Цель</title><p>Цель. Проведен ретроспективный анализ 10 ДТЛ за период с 01.2012 г. по 01.2014 г. </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В зависимости от этапа ДТЛ, на котором начата центральная веноартериальная экстракорпоральная мембранная оксигенация (в-а ЭКМО), было выделено две группы пациентов. Группа I (n = 4): начало ЭКМО после реперфузии первого или второго трансплантата в связи с развитием его отека. Группа II (n = 6): начало ЭКМО в ходе пневмонэктомии в связи с развитием тяжелой гипоксии (РаО2/FiO2 &lt; 1) и/или ацидоза (pH &lt; 7,2) во время ОИВЛ (группа II). </p></sec><sec><title>Результаты</title><p>Результаты. Длительность ЭКМО интраоперационно в группе I составила 4,1 ± 1,0 ч. У трех пациентов процедура была пролонгирована на 42 ± 6 ч в послеоперационном периоде в связи с первичной дисфункций трансплантатов. Одна пациентка из данной группы была переведена на периферическое вено-венозное ЭКМО. В группе II длительность в-а ЭКМО составила 8,5 ± 0,7 ч. Все пациенты этой группы были деканюлированы в конце трансплантации. Более раннее подключение в-а ЭКМО у пациентов группы II позволило стабилизировать газообмен и гемодинамику, снизить давление в легочной артерии во время реперфузии трансплантированных легких. За счет дренирования малого круга кровообращения среди пациентов группы II не было случаев отека трансплантата. Выживаемость в течение года составила 75 и 83,3% соответственно в группе I и II. </p></sec><sec><title>Выводы</title><p>Выводы. Центральная в-а ЭКМО является адекватным методом протезирования кардиореспираторной функции организма на всех этапах двусторонней трансплантации легких, предотвращает развитие отека трансплантированных легких за счет дренирования правых отделов сердца и управляемой гиподинамической реперфузии.</p></sec></abstract><trans-abstract xml:lang="en"><p>The most reliable intraoperative mechanical extracorporeal support is conventional сardiopulmonary bypass (CPB). However, CPB increases a risk of intraoperative bleeding and primary graft dysfunction. ECMO is more benefi cial method of intraoperative cardiopulmonary support than CPB in LTx.</p><sec><title>Aim</title><p>Aim. 10 LTx were retrospectively analyzed in the period from 01.2012 till 01.2014.</p></sec><sec><title>Methods</title><p>Methods. Indications for ECMO were acute grafts edema after reperfusion (n = 4, group I). In group II (n = 6) indications for ECMO were severe hypoxia (РаО2 and FiO2 ratio &lt; 1,0) and/or acidosis (pH &lt; 7,2) during one lung ventilation. We used central type of veno-arterial ECMO: right atrium to ascending aorta.</p></sec><sec><title>Results</title><p>Results. Intraoperative ECMO lasted 4,1 ± 1,0 hours in group I and 8,5 ± 0,7 hours in group II. ECMO was prolonged into postoperative period in all patients from group I due to primary graft dysfunction. Application of ECMO in group II enabled to stabilize gas exchange and circulation as well as to decrease pulmonary arterial pressure in the time of reperfusion. ECMO was fi nished just after transplantation in group II. The 1-year survival in group I and II was 75,0 and 83,3%, respectively.</p></sec><sec><title>Conclusion</title><p>Conclusion. Central veno-arterial ECMO is an adequate method of intraoperative cardiopulmonary support in LTx.
