Поражение почечного трансплантата при рецидиве ранее не диагностированной множественной миеломы (ММ) встречается редко. Представлено клиническое наблюдение за пациентом, перенесшим трансплантацию почки, у которого через год после операции развилась дисфункция трансплантата вследствие рецидива недиагностированной ММ.</p></sec><sec><title>Клиническое наблюдение</title><p>Клиническое наблюдение. Пациент 75 лет с артериальной гипертензией и сахарным диабетом самотеком поступил в отделение диализа с явлениями уремии. У нефролога не наблюдался. При ультразвуковом исследовании почек выявлены признаки диффузных изменений паренхимы. С учетом двух заболеваний диагностирована хроническая болезнь почек С5 в исходе гипертонической и диабетической нефропатии. Проводилась терапия гемодиализом. Через 11 мес. выполнена трансплантация почки от посмертного донора. Через год после операции развилась дисфункция трансплантата, выполнена его биопсия. Морфологическая картина наиболее характерна для поражения почек при ММ: каст-нефропатия (LCCN/Kappa) в сочетании с проксимальной тубулопатией легких цепей (LCPT/Kappa). Фокальный сегментарный гломерулосклероз. Тубулоинтерстициальный нефрит, острый тубулярный некроз. После дополнительного обследования диагностирована ММ с протеинурией Бенс-Джонса, III B стадия; миеломная нефропатия трансплантата (сочетание каст-нефропатии с проксимальной тубулопатией легких цепей). Пациент переведен в гематологическое отделение для проведения химиотерапии. Достигнута частичная гематологическая ремиссия. Функция почечного трансплантата не восстановилась, продолжено лечение программным гемодиализом.</p></sec><sec><title>Заключение</title><p>Заключение. Поражение трансплантированной почки вследствие ММ является редкой клинической ситуацией. В реальной клинической практике перед трансплантацией почки важно тщательное обследование пациентов и установление этиологии хронической болезни почек.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Kidney graft failure due to recurrence of previously undiagnosed multiple myeloma (MM) is a rare event. This report presents a clinical case of kidney transplantation complicated by graft dysfunction one year after surgery caused by recurrence of undiagnosed MM.</p></sec><sec><title>Clinical observation</title><p>Clinical observation. A 75-year-old man with a history of arterial hypertension and diabetes mellitus was admitted to the dialysis unit with symptoms of uremia. He had not previously been followed by a nephrologist. Renal ultrasonography revealed diffuse parenchymal changes. Considering the two diseases, chronic kidney disease (CKD C5) secondary to hypertensive and diabetic nephropathy was diagnosed, and maintenance hemodialysis was initiated. After 11 months, the patient underwent deceased-donor kidney transplantation. One year post-transplantation, graft dysfunction developed, prompting a transplant kidney biopsy. Histopathological examination revealed changes characteristic of MM-associated kidney injury, including light-chain cast nephropathy (LCCN, κ type) combined with light-chain proximal tubulopathy (LCPT, κ type), focal segmental glomerulosclerosis, tubulointerstitial nephritis, and acute tubular necrosis. Further evaluation confirmed MM with Bence–Jones proteinuria, stage III B, and myeloma nephropathy of the renal allograft. The patient was transferred to the hematology department for chemotherapy, resulting in partial hematologic remission. However, renal graft function was not restored, and the patient remained on maintenance hemodialysis.</p></sec><sec><title>Conclusion</title><p>Conclusion. MM-associated kidney injury is a rare clinical event.
