На разработанном в ГКБ им. С.П. Боткина устройстве проведена нормотермическая машинная перфузия правой почки от донора с расширенными критериями. Для перфузии использовался раствор на основе крови донора с добавлением раствора Рингера лактата и человеческого альбумина. Температура в контуре поддерживалась на уровне 37 °С. Время перфузии составило 203 минуты, по истечении которого индекс резистентности трансплантата снизился практически вдвое – с 0,33 до 0,16. Получено 120 мл мочи.</p></sec><sec><title>Результаты</title><p>Результаты. После окончания перфузии правая почка была успешно трансплантирована. В послеоперационном периоде отмечается немедленная функция правого почечного трансплантата. Креатинин сыворотки крови реципиента в 1-е сутки после трансплантации составил 530 мкмоль/л, при выписке на 14-е сутки – 170 мкмоль/л. Левая почка консервирована статическим холодовым способом и далее пересажена реципиенту.</p></sec><sec><title>Заключение</title><p>Заключение. Применение нормотермической перфузионной методики ex vivo с целью сохранения трансплантатов от доноров с расширенными критериями безопасно и осуществимо в клинической практике. Требуются дальнейшие исследования для определения четких показаний к ее использованию и формирования оптимального протокола выполнения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to assess if normothermic ex vivo machine perfusion (NMP) of a kidney from an expanded criteria donor (ECD) is feasible and safe.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. NMP of the right kidney from an ECD was performed on a device developed at Botkin Hospital. A solution based on donor’s blood with the addition of Ringer’s lactate solution and human albumin was used for perfusion. The temperature in the circuit was maintained at 37 °C. Perfusion lasted for 203 minutes, after which the renal resistive index was almost halved from 0.33 to 0.16. 120 ml of urine was obtained.</p></sec><sec><title>Results</title><p>Results. The right kidney was successfully transplanted after perfusion. There was immediate function of the right renal graft in the postoperative period. The recipient’s serum creatinine level was 530 μmol/L on day 1 following transplantation and 170 μmol/L on day 14 of discharge. The left kidney was preserved by static cold storage and further transplanted to the recipient.</p></sec><sec><title>Conclusion</title><p>Conclusion. The use of NMP to preserve grafts obtained from ECDs is safe and feasible in clinical practice.
