На сегодняшний день нет единых подходов к вопросам хирургического лечения и подготовки к трансплантации данной категории пациентов.</p><sec><title>Цель</title><p>Цель: проанализировать пятнадцатилетний опыт работы трансплантационного центра с реципиентами, у которых терминальная стадия хронической болезни почек (тХБП) обусловлена АДПП.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. Проведены ретроспективный и проспективный анализы этапного хирургического лечения 132 пациентов с АДПП в период с 2008-го по 2023 год. На первом этапе проанализированы результаты выполнения 155 нефрэктомий ПКП лапароскопическим и открытым доступами. Вторым этапом изучены результаты трансплантации почки 63 реципиентам с АДПП с сохраненными или удаленными нативными почками. В качестве группы сравнения проанализированы результаты трансплантации почки 129 пациентам с тХБП другой этиологии за период с 2013-го по 2023 г.</p></sec><sec><title>Результаты</title><p>Результаты. Выявлены значимые преимущества лапароскопического доступа для выполнения нефрэктомии ПКП, проявляющиеся в снижении длительности пребывания пациента в реанимации и в стационаре, снижении частоты осложнений (47,8% для лапаротомного и люмботомического доступа, 12,8% в случаях лапароскопического доступа). Отмечено повышение частоты инфекционных осложнений (до 26,9%) после трансплантации почки у пациентов с сохраненными ПКП за счет инфицирования кист и присутствия у таких пациентов резистентной к стандартным схемам антибиотикопрофилактики инфекции. Десятилетняя выживаемость трансплантатов в группе с сохраненными ПКП составила 46,2%, для группы с удаленными ПКП и группы сравнения этот показатель составил соответственно 73,1 и 74,1%.</p></sec><sec><title>Выводы</title><p>Выводы. Внедрение в клиническую практику лапароскопической хирургии ПКП позволяет значимо снизить частоту хирургических осложнений и расширить показания к нефрэктомии ПКП. Среди пациентов с АДПП, которым выполнена нефрэктомия ПКП, отмечается более благоприятное течение посттрансплантационного периода, и результаты трансплантации почки таким реципиентам не отличаются от таковых в группе больных с тХПН другой этиологии.</p></sec></abstract><trans-abstract xml:lang="en"><p>Treatment and kidney transplantation (KT) for patients with autosomal dominant polycystic kidney disease (ADPKD) are associated with increased risks, particularly due to the potential for infection of polycystic kidney (PK) cysts. Currently, no standardized guidelines exist for the surgical management and pre-transplant preparation of these patients.</p><sec><title>Objective</title><p>Objective: to analyze the 15-year experience at a transplant center managing KT recipients with end-stage chronic kidney disease (eCKD) due to ADPKD.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A retrospec- tive and prospective analysis was conducted on 132 ADPKD patients who underwent staged surgical treatment between 2008 and 2023. In the first stage, outcomes of 155 PK nephrectomies performed via laparoscopic and open approaches were evaluated. In the second stage, KT outcomes were assessed in 63 ADPKD recipients, com- paring those with preserved native kidneys to those who had undergone nephrectomy. Additionally, as a control group, KT outcomes in 129 patients with eCKD of other etiologies from 2013 to 2023 were analyzed.</p></sec><sec><title>Results</title><p>Results. The study revealed significant advantages of laparoscopic access for PK nephrectomy, including a shorter length of stay in both intensive care and the hospital, as well as a lower complication rate (47.8% for laparotomy and lumbotomy approaches, and 12.8% for laparoscopic access). However, patients who underwent KT with preser- ved PK exhibited a higher incidence of infectious complications (26.9%), primarily due to cyst infections and resistance to standard antibiotic prophylaxis. Long-term graft survival was notably lower in this group, with a ten-year survival rate of 46.2%, compared to 73.1% in patients who had undergone nephrectomy and 74.1% in the comparison group.</p></sec><sec><title>Conclusion</title><p>Conclusion. The integration of laparoscopic surgery for polycystic kidney disease into clinical practice has the potential to significantly reduce surgical complications and broaden the indications for PK nephrectomy.
