Проведено сравнительное ретроспективное исследование у 54 пациентов, находившихся в листе ожидания трансплантации печени в период с 2017-го по 2023 г., перенесших повторные кровотечения из варикозных вен пищевода после проведения вторичной профилактики кровотечений до включения в исследование. Проведен анализ демографических, клинических и лабораторных показателей, клинических индексов, печеночной энцефалопатии, выраженности асцита, степени варикозных узлов, манометрического исследования до и после имплантации TIPS с эмболизацией желудочных вен, с расчетом градиента портального давления у больных с повторными кровотечениями (n = 16) и без них (n = 38). Методом Каплана–Майера с определением логарифмического критерия (Log-Rank) проведено сравнение пропорций пациентов. Оценку накопленных рисков проводили c помощью одновариантного и многовариантного анализа модели пропорциональных рисков Кокса.</p></sec><sec><title>Результаты</title><p>Результаты. В течение 30 недель от даты TIPS в сочетании с эмболизацией желудочных вен у 16 из 54 больных (29,6%) развились повторные кровотечения. Выявлены факторы риска: показатели возраста, концентраций гемоглобина, лейкоцитов, тромбоцитов, креатинина, степени асцита, среднего значения уровня градиента портального давления после имплантации TIPS. Установлено, что пропорция пациентов без кровотечений была значимо большей у больных с величиной градиента портального давления ≤10 мм рт. ст., чем при величине этого показателя &gt;10 мм рт. ст. (Log Rank = 0,029). Определены независимые предикторы повторных кровотечений: выраженность асцита, тромбоз шунта, показатель градиента портального давления после имплантации TIPS, показатель градиента портального давления после имплантации TIPS &lt;30% от базального уровня. Показано, что риск развития повторных кровотечений при градиенте портального давления &gt;10 мм рт. ст. прогрессивно нарастает и достигает максимальных значений (HR = 1,713) у пациентов, подвергшихся TIPS в сочетании с эмболизацией желудочных вен в сроки от 32 до 40 недель от времени операции, в то время как при градиенте портального давления ≤10 мм рт. ст. он отсутствует.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to determine the predictors and risk of recurrent bleeding after implantation of a transjugular intrahepatic portosystemic shunt (TIPS) combined with selective gastric vein embolization in patients with decompensated cirrhosis awaiting liver transplantation (LT).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A comparative retrospective study was performed in 54 patients waitlisted for LT between 2017 and 2023, who suffered recurrent variceal hemorrhage after secondary prophylaxis of bleeding prior to inclusion in the study. Demographic, clinical and laboratory parameters, clinical indices, hepatic encephalopathy, severity of ascites, degree of varices, manometric study before and after TIPS implantation with gastric vein embolization, with calculation of portal pressure gradient in patients with (n = 16) and without rebleeding (n = 38), were analyzed. The proportions of patients were compared using the Kaplan–Meier method with determination of the logarithmic test (Log-Rank). Cumulative risks were estimated by means of univariate and multivariate analysis of the Cox proportional hazards model.</p></sec><sec><title>Results</title><p>Results. Within 30 weeks from the date of TIPS combined with gastric vein embolization, 16 of 54 patients (29.6%) developed rebleeding. The following risk factors were identified: age, hemoglobin level, white blood cell count, platelet count, creatinine level, severity of ascites, and mean portal pressure gradient after TIPS implantation. It was found that the proportion of patients without bleeding was significantly higher in patients with portal pressure gradient ≤10 mmHg than in patients with this index &gt;10 mmHg (Log Rank = 0.029). The following independent predictors of recurrent hemorrhage were determined: severity of ascites, shunt thrombosis, portal pressure gradient after TIPS implantation, portal pressure gradient after TIPS implantation &lt;30% of the basal level.
