Трансплантация печени от живого донора является эффективным методом лечения пациентов с различными видами терминальных заболеваний печени.</p></sec><sec><title>Цель</title><p>Цель: оценить результаты первых самостоятельно проведенных трансплантаций печени от живого донора в РНПМЦХ им. В. Вахидова на базе отделения гепатобилиарной хирургии и показать, что в условиях нашей клиники трансплантация печени является выполнимой процедурой.</p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. С октября 2021-го по декабрь 2023 года в нашем отделении было проведено 40 трансплантаций правой доли печени от живого донора. Оценивались краткосрочные и отдаленные результаты у реципиентов. Также были оценены результаты донорских резекций печени.</p></sec><sec><title>Результаты</title><p>Результаты. В 1 случае (2,5%) развился тромбоз печеночной артерии. В 3 случаях (7,5%) были диагностированы стенозы артериальных анастомозов, которые были устранены эндоваскулярным баллонным расширением. В 3 случаях (7,5%) развился синдром обкрадывания селезеночной артерией; он был разрешен эндоваскулярной эмболизацией селезеночной артерии. У одного пациента (2,5%) развился тромбоз воротной вены. У 2 пациентов (5%) через 10 месяцев после трансплантации был выявлен стеноз воротной вены; была проведена эндоваскулярная баллонная ангиопластика с хорошим клиническим эффектом. Желчные осложнения составили 45%, из которых 89% были желчеистечения, а 11% – стриктуры желчных анастомозов. Госпитальная летальность составила 12,5%.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты нашего опыта и анализ пост-трансплантационных осложнений сопоставимы с данными мировой литературы и приемлемы на этапе внедрения программы трансплантации печени. Трансплантация выполнима в условиях нашего центра, однако необходимо улучшать хирургические и консервативные методы лечения с целью минимизации развития как ранних, так и отдаленных послеоперационных осложнений.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background. Living-donor liver transplant (LDLT) is a life-saving procedure for patients with end-stage liver diseases. </p></sec><sec><title>Objective</title><p>Objective: to evaluate the outcomes of the first independent LDLT performed at the Department of Hepatobiliary Surgery, Vakhidov Republican Specialized Surgical Research and Practical Medical Center of Surgery, and to demonstrate that liver transplantation (LT) is a feasible procedure at our institution.</p></sec><sec><title>Materials and methods</title><p>Materials and methods. From October 2021 to December 2023, 40 right lobe LDLTs were performed in our department. Short-term and long-term outcomes in recipients were assessed. The outcomes of transplant hepatectomy were also evaluated.</p></sec><sec><title>Results</title><p>Results. Hepatic artery thrombosis developed in 1 case (2.5%); arterial anastomotic stenosis was detected in 3 cases (7.5%), which were repaired by endovascular balloon dilation; splenic artery steal syndrome was diagnosed in 3 cases (7.5%), which was resolved by endovascular splenic artery embolization. One patient (2.5%) developed portal vein thrombosis. Two patients (5%) had portal vein stenosis 10 months after transplantation; endovascular balloon angioplasty was performed with good clinical effect. Biliary complications accounted for 45%, of which 89% were biliary leaks and 11% were anastomotic biliary stricture. In-hospital mortality was 12.5%.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of our experience and analysis of post-transplant complications are comparable with those of the world literature and are acceptable at the stage of implementation of the LT program.
