Проведено сравнительное ретроспективное исследование пациентов с декомпенсированным циррозом (n = 51), находившихся в листе ожидания трансплантации печени. Проведен анализ демографических, клинических и лабораторных показателей, индексов MELD-Na, Child–Turcotte–Pugh, печеночной энцефалопатии, выраженности асцита, степени варикозных узлов, количества последовательных лигирований варикозных узлов, а также манометрического исследования с расчетом показателя градиента внутрипеченочного венозного давления в группах пациентов с развившимися повторными кровотечениями (n = 39) и без них (n = 12). Методом Каплана–Майера с определением логарифмического критерия (Log-Rank) проведено сравнение пропорций пациентов в различных группах. Оценку накопленных рисков в сравниваемых группах проводили c помощью математической модели пропорциональных рисков (регрессии Кокса) в одновариантном и многовариантном анализе.</p></sec><sec><title>Результаты</title><p>Результаты. В течение 60 месяцев от начала наблюдения и одновременного начала профилактики посредством комбинации приема неселективных β-блокаторов и эндоскопического лигирования варикозных узлов у 39 из 51 больного (75,6%) развились повторные кровотечения. На основе проведенного анализа выявлены значимые различия (факторы риска развития повторных кровотечений): показатели концентраций креатинина, MELD-Na, степени печеночной энцефалопатии, среднего уровня градиента печеночного венозного давления (ГПВД) и его уровня &gt;14 мм рт. ст. Методом Каплана–Майера с определением Log-Rank-критерия установлено, что пропорция пациентов без развития повторных кровотечений была достоверно большей в группе больных с величиной ГПВД ≤14 мм рт. ст., чем в группе больных с величиной ГПВД &gt;14 мм рт. ст. (р = 0,027).</p></sec><sec><title>Заключение</title><p>Заключение. Показано, что основным независимым предиктором развития повторных кровотечений является величина ГПВД &gt;14 мм рт. ст., которая при изменении значения градиента на 1 мм увеличивает риск в 3,837 раза. Вторым независимым предиктором является увеличение выраженности печеночной энцефалопатии: при увеличении показателя ее степени на единицу риск повторных кровотечений увеличивается в 1,8 раза.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Objective</title><p>Objective: to identify the risk factors and predictors of recurrent variceal hemorrhage in cirrhotic patients awaiting liver transplantation (LT).</p></sec><sec><title>Materials and methods</title><p>Materials and methods. A comparative retrospective study was conducted in 51 patients with decompensated cirrhosis, who were on the waiting list for LT. Demographic, clinical and laboratory parameters, MELD-Na score, Child–Turcotte–Pugh score, hepatic encephalopathy grade, ascites grade, class of varicose veins, number of consecutive variceal ligations, as well as manometric study with calculation of intrahe- patic venous pressure gradient index in groups of patients with (n = 39) and without recurrent bleeding (n = 12) were analyzed. The proportions of patients in different groups were compared by the Kaplan–Meier method with determination of the logarithmic test (Log-Rank). The accumulated risks in the compared groups were estimated using the mathematical model of proportional hazards (Cox regression) in univariate and multivariate analysis.</p></sec><sec><title>Results</title><p>Results. Within 60 months from the beginning of follow-up and simultaneous prophylaxis by combination of non-selective beta-blockers and endoscopic variceal ligation (EVL), 39 out of 51 patients (75.6%) developed recurrent bleeding. Analysis revealed significant differences (risk factors for recurrent bleeding): creatinine le- vels, MELD-Na score, hepatic encephalopathy grade, mean hepatic venous pressure gradient (HVPG) and its level &gt;14 mmHg. By the Kaplan–Meier method with the Log-Rank test, it was established that the proportion of patients without recurrent bleeding was significantly higher in the group of patients with HVPG ≤14 mmHg than in the group with HVPG &gt;14 mmHg (p = 0.027).</p></sec><sec><title>Conclusion</title><p>Conclusion. The main independent predictor of variceal rebleeding is HVPG &gt;14 mm Hg, which increases the risk by 3.837 times if the gradient value is changed by 1 mm.
