Цель: представить клиническое наблюдение ребенка 11 лет, перенесшего повторную трансплантацию сердца в ФГБУ «НМИЦ им. В.А. Алмазова» Минздрава России.

Материалы и методы

Материалы и методы. Описан случай успешной ретрансплантации сердца у ребенка 11 лет с васкулопатией сердечного аллотрансплантата.

Результаты

Результаты. Течение послеоперационного периода у пациента после ретрансплантации сердца не имело существенных различий с послеоперационным периодом первичных реципиентов сердца. Сложность интраоперационного этапа определялась выраженным спаечным процессом. В рамках предоперационной подготовки пациенту была выполнена КТ грудной клетки, что, по нашему опыту, позволяет оценить синтопию сердца и в свою очередь является важным подготовительным этапом при планировании повторных вмешательств.

Заключение

Заключение. Наш первый опыт ретрансплантата сердца у пациентов детской возрастной группы говорит о том что повторная трансплантация сердца подходит как наиболее оптимальный метод лечения у пациентов с васкулопатией сердечного аллотрансплантата и требует более тщательной предопе рационной подготовки.

Objective

Objective: to present a clinical case of an 11-year-old child who underwent repeat heart transplantation (HT) at Almazov National Medical Research Centre in St. Petersburg, Russia.

Materials and methods

Materials and methods. A case of successful heart retransplantation in an 11-year-old child with cardiac allograft vasculopathy (CAV) is presented.

Results

Results. The postoperative period after heart retransplantation had no significant differences with the postoperative period of primary heart recipients. The complexity of the intraoperative stage was determined by pronounced adhesions. As part of preoperative preparation, the patient underwent chest CT scan, which, in our experience, allows us to evaluate the heart syntopy and, in turn, is an important preparatory stage in planning repeat interventions.

Conclusion

Conclusion. Our first experience of cardiac retransplantation in pediatric patients suggests that repeat HT is the most optimal treatment for pediatric patients with CAV and requires more thorough preoperative preparation.
