Использование для транс- плантации органов, полученных от доноров с расширенными критериями, позволяет расширить донор- ский пул, увеличивая доступность трансплантации нуждающимся в ней пациентам с терминальными стадиями болезней внутренних органов. Основным препятствием к широкому использованию доноров с расширенными критериями является ишемическое повреждение, следующая за ним ишемически- реперфузионная травма органов, активация сценариев программируемой клеточной гибели, приводящие к снижению функционального резерва трансплантатов, а зачастую к развитию тяжелых осложнений в послеоперационном периоде. Применение антисмысловой генной терапии при трансплантации может послужить терапевтическим инструментом, позволяющим влиять на трансплантат на всех этапах работы с ним, улучшая его качество, и следовательно, результат трансплантации. </p></abstract><trans-abstract xml:lang="en"><p>Global organ shortage is the crucial point of transplantation nowadays. Usage of expanded criteria donors represents reliable source of donor organs, making transplantation more accessible for patients with end stage organs failure. Ischemia-reperfusion injury followed by the activation of programmed cell death scenarios remains the main obstacle in utilization of marginal grafts. Programmed cell death often leads to life threatening complications in posttransplant period.
