Трансплантации сердца (ТС) у пациентов с предсуществующим сахарным диабетом (СД) 2-го типа сопряжена с повышенными риском инфекционных и неинфекционных (почечная дисфункция, мультифокальный атеросклероз, болезнь коронарных артерий пересаженного сердца и т. п.) осложнений, которые могут негативно повлиять на выживаемость реципиентов в ранние и отдаленные сроки после ТС.</p></sec><sec><title>Цель исследования</title><p>Цель исследования. На основании одноцентрового ретроспективного исследования оценить влияние предтрансплантационого СД 2-го типа на ранние и отдаленные результаты ТС. </p></sec><sec><title>Материалы и методы</title><p>Материалы и методы. В исследование включили реципиентов (n = 891), которым была выполнена ТС в период с 2011-го по 2018 г. Реципиенты были разделены на две группы: основная группа (группа «СД2») – реципиенты с дотрансплантационным СД 2- го типа (n = 80; 9,0%), контрольная группа (группа «без СД2») – реципиенты без СД 2-го типа (n = 811; 91,0%). Реципиенты обеих групп не различались по неотложности ТС  (UNOS статус) и потребности в предтрансплантационной механической поддержке кровообращения (МПК). </p></sec><sec><title>Результаты</title><p>Результаты. На момент выполнения ТС реципиенты группа «СД2» по сравнению с реципиентами группы «без СД2» были старше по возрасту (54 [46; 59] года против 48 [35; 56] лет, p &lt; 0,001), имели больший вес (p &lt; 0,001) и индекс массы тела (p &lt; 0,001), основным заболеванием чаще была ишемическая болезнь сердца (65,0% против 36,5%, p &lt; 0,001), имели более высокие значения транспульмонального градиента (10,0  [7,0; 12,0] против 9,0 [6,0; 12,0] мм рт. ст., р = 0,024) и легочного сосудистого сопротивления (2,9 [2,2; 4,0] против 2,5 [1,8; 3,4] ед. Вуда, p = 0,038). Перед ТС реципиенты группы «СД2» имели выраженные проявления почечной дисфункции и повышенную коморбидность. Реципиенты обеих групп не различались по  характеристикам донора сердца, продолжительности ишемии трансплантата и искусственного кровообращения, частоте возникновения выраженной ранней дисфункции сердечного трансплантата, потребовавшей МПК (12,5% против 10,7%, р = 0,74). У реципиентов группы «СД2» в раннем периоде после ТС большие потребность (100% против 28,0%, p &lt; 0,001) и дозировки инсулинотерапии, более выраженные проявления почечной дисфункции и большая потребность в заместительной почечной терапии (45,0% против 27,9%, р = 0,003) не повлияли на продолжительность искусственной вентиляции легких, длительность лечения в ОРИТ (6 [5; 10] дней против 6 [5; 10] дней, p = 0,098) и госпитальную летальность (8,8%  против 8,5%, p = 0,895). Наличие дотрансплантационного СД 2-го типа не оказало отрицательного влияния на частоту возникновения острого отторжения сердечного  трансплантата, прогрессирование трансмиссивного атеросклероза коронарных артерий, частоту возникновения и выраженность васкулопатии сердечного трансплантата, на структуру и выраженность отдаленных инфекционных и неинфекционных осложнений и посттрансплантационную выживаемость. </p></sec><sec><title>Заключение</title><p>Заключение. При правильном отборе реципиентов и выборе оптимальной тактики их ведения в посттрансплантационном периоде наличие дотрансплантационного СД 2-го  типа не оказывает отрицательного влияния на ранние и отдаленные результаты ТС.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Introduction</title><p>Introduction. Heart transplantation (HT) in patients with preexisting type 2 diabetes (T2D) is associated with high risk of infectious and non-infectious complications (renal dysfunction, multifocal atherosclerosis,  transplant coronary artery disease, etc.) that can negatively affect recipient survival in the early and late periods after HT.</p></sec><sec><title>Objective</title><p>Objective: to assess the effect of pre-transplant T2D on early and long-term outcomes of HT based on a  singlecenter retrospective study. </p></sec><sec><title>Materials and methods</title><p>Materials and methods. The study enrolled 891 recipients who underwent HT within the period 2011 to 2018, and were divided into two groups: main group (T2D) – recipients with pretransplant T2D (n = 80, 9.0%) and the control group (T2D-free) – recipients without T2D (n = 811, 91.0%). Recipients from both groups did not differ in terms of HT urgency (UNOS status) and the need for pre-transplant mechanical circulatory support (MCS). </p></sec><sec><title>Results</title><p>Results. At the time of the HT, recipients from the T2D group were older than the T2D-free recipients (54 [46; 59] years vs 48 [35; 56] years, p &lt; 0.001), they had a higher weight (p &lt; 0.001) and body mass index (p &lt; 0.001), coronary heart disease was more often their main disease (65.0% vs 36.5%, p &lt; 0.001), they had higher transpulmonary gradient (10.0 [7.0; 12.0] mm Hg vs 9.0 [6.0; 12.0] mm Hg, p = 0.024) and  pulmonary vascular resistance (2.9 [2.2; 4.0] Wood units vs 2.5 [1.8; 3.4] Wood units, p = 0.038). In the pre-transplant period, the T2D group had pronounced manifestations of renal dysfunction and increased comorbidity. Recipients in both groups did not differ in terms of cardiac donor parameters, graft ischemia time, cardiopulmonary bypass time, and incidence of severe early heart graft dysfunction requiring MCS (12.5% vs 10.7%, p = 0.74). In the early post-transplant period, the T2D group had high requirements (100% vs 28.0%, p &lt; 0.001) and higher doses of insulin therapy. More pronounced manifestations of renal dysfunction and a greater need for renal replacement therapy (51.4% vs 27.9%, p = 0.003) did not affect artificial ventilation and ICU duration (6 [5; 10] days vs 6 [5; 10] days, p = 0.098), as well as hospital mortality ( 8.8% vs 8.5%, p = 0.895). The presence of pre-transplant T2D had no negative effect on the incidence of acute cardiac graft rejection, progression of transmissible coronary atherosclerosis, incidence and severity of cardiac graft vasculopathy, structure and severity of distant infectious and non-infectious complications, and post-transplant survival. </p></sec><sec><title>Conclusion</title><p>Conclusion.
