Это заболевание включает множество генетических, метаболических и экологических факторов. Оно тесно связано с инсулинорезистентностью, метаболическим синдромом, ожирением, сахарным диабетом и многими другими заболеваниями. Неалкогольная жировая болезнь печени характеризуется макровезикулярным стеатозом печени. При естественном течении НАЖБП простой стеатоз прогрессирует до неалкогольного стеатогепатита (НАСГ), фиброза и в конечном счете до цирроза и гепатоцеллюлярной карциномы. Цирроз при НАСГ и гепатоцеллюлярная карцинома являются показанием для трансплантации печени. Ожирение является растущей проблемой у кандидатов на трансплантацию печени. Сердечно-сосудистые осложнения, связанные с метаболическим синдромом, и рецидив НАСГ в трансплантированной печени могут повлиять на исход операции у этих пациентов. Результаты после трансплантации схожи с результатами трансплантации печени по другим показаниям, но сердечно-сосудистые осложнения являются основной причиной смерти больных с НАСГ после операции.</p></abstract><trans-abstract xml:lang="en"><p>Nonalcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease in many countries, involving about 25% of the population worldwide. This disease includes many genetic, metabolic, and environmental factors. It is closely associated with insulin resistance, metabolic syndrome, obesity, diabetes, and many other diseases. NAFLD is characterized by macrovesicular steatosis of the liver. In the natural course of NAFLD simple steatosis progresses to nonalcoholic steatohepatitis (NASH), fibrosis and ultimately, cirrhosis and hepatocellular carcinoma. Cirrhosis with Nash and hepatocellular carcinoma is an indication for liver transplantation. Obesity is a growing problem in liver transplant candidates. Cardiovascular complications related to metabolic syndrome and NASH recurrence in the transplanted liver may affect the outcome of surgery in these patients.
