Для решения поставленных задач в исследование включено 150 пациентов, которым была выполнена ортотопическая трансплантация сердца в Федеральном государственном бюджетном учреждении «Национальный медицинский исследовательский центр трансплантологии и искусственных органов имени акад. В.И. Шумакова» Министерства здравоохранения Российской Федерации в период с января 2009 г. по март 2018 г. В качестве основного метода оценки качества жизни был использован опросник качества жизни «SF-36» (MedicalQutcomesStude 36 - ItemShortFormheartsurgery).</p></sec><sec><title>Результаты</title><p>Результаты. Анализ результатов исследования свидетельствует о крайне низких показателях как физической активности, так и оценки общего состояния здоровья на предтрансплантационном этапе. В течение первого месяца после трансплантации сердца существенно возрастают ролевое функционирование, обусловленное физическим состоянием (р = 0,02), общее состояние здоровья (р = 0,001), психическое здоровье (р = 0,01) и жизненная активность (р = 0,01). Толерантность к физической нагрузке возрастает на 201,26% в течение первого месяца после ТС (р = 0,02). Наиболее полно показатели качества жизни восстанавливаются через один год после операции: физическое функционирование (р = 0,00007), ролевое функционирование, обусловленное физическим состоянием (р = 0,00006), болевой порог (р = 0,04), общее состояние здоровья (р = 0,00007), психическое здоровье (р = 0,0001), ролевое функционирование, обусловленное эмоциональным состоянием (р = 0,01), социальное функционирование (р = 0,0001), жизненная активность (р = 0,000032). Высокие показатели качества жизни сохраняются в сроки более 5 лет после трансплантации. Сравнительный анализ результатов, полученных в предоперационном периоде и спустя год после проведения операции ортотопической трансплантации сердца свидетельствует о значительном улучшении качества жизни пациентов.</p></sec><sec><title>Заключение</title><p>Заключение. Результаты исследования показали, что в течение года после трансплантации сердца у больных терминальной сердечной недостаточностью отмечается значительное и достоверное улучшение всех показателей, характеризующих качество жизни. Высокие уровни показателей физической, психической и социальной активности, достигнутые к первому году, сохраняются как минимум на протяжении 5 лет, что свидетельствует о высокой эффективности операции ортотопической трансплантации сердца как радикального метода лечения тяжелой сердечной недостаточности.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Aim</title><p>Aim: the aim of the study was a comparative assessment of the quality of life of recipients with donor heart at the stages before and after heart transplantation.</p></sec><sec><title>Materials and methods</title><p>Materials and methods: the study included 150 patients who underwent orthotopic heart transplantation at the «Academician V.I. Shumakov National Medical Research Center of Transplantology and Artificial Organs», Ministry of Health of the Russian Federation from January 2009 to March 2018.</p></sec><sec><title>Results</title><p>Results. Analysis of the research results indicates extremely low rates of both physical activity and overall health assessment at the pre-transplantation stage. During the first month after heart transplantation, the role of functioning due to physical condition (p = 0.02), general health (p = 0.001), mental health (p = 0.01) and vital activity (p = 0.01) significantly increase. Tolerance to physical activity increases by 201.26% during the first month after the heart transplantation (p = 0.02). The most complete indicators of quality of life are restored after one year of operation: physical functioning (p = 0.00007), role-based functioning due to physical condition (p = 0.00006), pain threshold (p = 0.04), general health (p = 0.00007), mental health (p = 0.0001), role functioning due to the emotional state (p = 0.01), social functioning (p = 0.0001), vital activity (p = 0.000032). High rates of quality of life are maintained for periods longer than 5 years after transplantation. The best indicators of quality of life in recipients of transplanted heart are observed a year after surgery and manifest a significant increase in all indicators. A comparative analysis of the results obtained in the preoperative period and a year after the operation of orthotopic heart transplantation indicates a significant improvement in the quality of life of patients.</p></sec><sec><title>Conclusion</title><p>Conclusion. The results of the study showed that during the year after heart transplantation in patients with terminal heart failure, there was a significant improvement in all indicators characterizing the quality of life.
